What happens if your pet injures someone or damages property in Spain?
Private health insurance in Spain covers your GP, specialists (no referral needed), dental, worldwide emergencies and surgery — from as little as €30/month. Compared to the UK, Germany and France, Spain offers broader coverage, shorter waiting times, and significantly lower premiums. It’s not just convenient — for most expats, it’s legally required.
Most expats moving to Spain make the same dangerous assumption: “my current cover will be fine.” It won’t. And by the time they find out, they’re sitting in a Spanish hospital — alone, confused, and facing a bill that can run into tens of thousands of euros.
The reality is brutal: if you’re not contributing to the Spanish Social Security system, you have no automatic right to public healthcare. No NHS. No European Health Card for anything beyond emergencies. Nothing. And if you’re applying for a Non-Lucrative Visa or Spanish residency, the law requires you to prove you have comprehensive private health insurance — or your application gets rejected, full stop.
Why your home country cover won’t protect you in Spain
Your UK private health insurance? It almost certainly doesn’t cover planned treatment in Spain. Your EHIC or GHIC card? That only covers emergency care — and only while you’re a tourist, not a resident. Once you register as a resident, that protection disappears.
German or French public cover? If you stop contributing to their system, your entitlement stops with it.
And the Spanish public system (SNS)? Excellent in emergencies — but for everything else, waiting times for specialists in some regions exceed 180 days. That’s six months to see a cardiologist. Six months to get a scan. Six months wondering.
The worst-case scenario is more common than you think:
- An expat skips private cover to save money
- They fall seriously ill or have an accident
- They’re not entitled to public healthcare
- They receive private emergency treatment they didn’t plan for
- The bill arrives: €15,000. €40,000. More.

What private health insurance in Spain actually covers
This is where Spain genuinely surprises people — in the best possible way. A comprehensive private policy here covers far more than most expats expect:
- GP consultations with no queues and no paperwork
- Direct access to specialists — no GP referral needed, no waiting months
- Hospitalisation and surgery at leading private hospitals
- Dental cover included in most policies (not an expensive add-on)
- Worldwide emergency medical cover
- Diagnostic tests: blood tests, X-rays, ultrasounds, MRIs
- Gynaecology — choose your own specialist
- Nutritionist and obesity treatment for adults and children
- Cancer diagnosis and treatment at the best medical centres in Spain
- No age limit — yes, you read that right
Pricing starts from €18/month with a co-payment or from €30/month with no co-payment — the latter being the only option valid for visa and residency applications.

Country by country: what you’re leaving behind
United Kingdom
The NHS is under immense pressure. As of 2024, average waiting times for elective surgery exceeded 18 weeks — and in some specialties, patients wait over a year. Dental care is not covered beyond basic NHS treatment. Seeing a specialist without a GP referral is simply not an option. British expats who move to Spain and take out private health insurance here consistently describe it as one of the best financial decisions they’ve made.
Germany
Germany operates a dual system where higher earners can opt into private insurance (PKV). But premiums increase sharply with age, and for expats who aren’t employed and contributing to the system, private insurance is mandatory — and significantly more expensive than equivalent cover in Spain.
France
France has one of the strongest public health systems in Europe, but it still requires a supplementary private policy (called a mutuelle) to cover co-payments, which add up quickly. Seeing a specialist without a GP referral is financially penalised. None of this applies when you have private health insurance in Spain.
The situations where no cover becomes a catastrophe
These are real scenarios. They happen every week to expats in Spain:
- Sudden cardiac event requiring hospitalisation and surgery: €25,000–€60,000 without cover
- Cancer diagnosis requiring chemotherapy and specialist treatment: costs that run into hundreds of thousands
- Road accident with serious injuries and extended ICU stay: no upper limit
- Visa or residency application rejected because the health insurance policy didn’t meet Spanish legal requirements
- Pre-existing condition excluded because it wasn’t declared at the time of taking out the policy
Every single one of these situations is avoidable. The right policy costs less per day than a coffee.
When does it make sense to take out health insurance in Spain rather than your home country?
Almost always. But especially if:
- You’re applying for a Non-Lucrative Visa or Spanish residency
- You’re retired and no longer covered by an employer’s policy
- Your existing cover doesn’t include planned treatment in Spain
- You want to see a specialist within days, not months
- You travel frequently and need worldwide emergency cover
- You’re over 65 and your current insurer won’t renew your policy
- You simply want peace of mind — and your family to stop worrying
Frequently asked questions
It depends on the insurer and the condition. Some exclude them; others cover them with an adjusted premium. What is non-negotiable: always declare them. Hiding a pre-existing condition can invalidate your entire policy — leaving you with a massive bill and no recourse.
Not with us. The Insurance Centre works with policies that have no age limit — something many insurers won’t offer.
Yes. Only policies without a co-payment are accepted for visa and residency applications. This is a detail that catches people out — make sure you confirm this before signing anything.
Emergency cover begins on day one. Other coverages such as planned surgery or specialist consultations may have waiting periods — unless you’re switching from one private insurer to another, in which case waiting periods are typically waived.
Many of our policies include worldwide emergency medical cover. For planned treatment outside Spain, premium worldwide policies cover 100% of costs with your choice of hospital or specialist anywhere in the world.
Don’t wait until something goes wrong
Here’s the uncomfortable truth: nobody thinks they’ll be the one who needs it urgently. Until they are.
The expats who call us in a panic are always the ones who put it off. “I’ll sort it next month.” “I’m healthy, I don’t need it yet.” “It seems expensive.” It isn’t expensive. What’s expensive is not having it.
At The Insurance Centre, we’ve been helping expats across Spain for over 30 years. We speak your language — literally — and we know every policy detail so you don’t have to.
